We launched amescosa Treatment Centre with its variety of services as outpatient program in January 2017 at Uwanja wa Water in Kilifi after an extensive research on the situation of youth affected by substance use disorders and existing support programs at the Coast of Kenya.
amescosa Reaching Out and Pre-Treatment support
Reaching out to young women and mothers in alcohol and poly-drug dependence in their immediate environment helps to give the necessary individual support to overcome the different barriers they are facing in accessing and entering treatment.
Pre-Treatment is the precursor for the dependence treatment, rehabilitation and holistic recovery process aiming to give in a safe, protected and empathic atmosphere positive stimulus and direction for fundamental transformation toward a healthy, self-responsible, self-determined and stable life.
Pre-Treatment engages young women and mothers in alcohol and polydrug dependence through direct self-involvement to minimize the vulnerability and risk factors including sex work, to foster protection, to highlight women and mothers specific concerns, to promote hygiene and health considering HIV and Aids, TB, Hepatitis C, reproductive health and healthy nutrition, to enlighten on the importance of contraceptives, condoms and family planing and to encourage a serious practice, to elicit self-responsibility, to initiate personal development and change of attitude, to offer individual adjusted coaching, care and therapy in respect of systemic work, to reduce the dosage of the substances, to apply homeopathic treatment and if necessary to refer to additional medical care and to find a meaningful occupation to secure the livelihood sustainability.
Pre-Treatment ensures that the process leading to the dependence treatment and rehabilitation is an active and self-responsible involvement and that the necessary space for measures to be taken in the immediate living environment is given, and organized and implemented beforehand to reach an effective and sustained recovery.
Pre-Treatment is low-threshold and the clients engage themselves intensively with essential topics and form the vital basis, on which the long-term recovery process can be built.
Treatment, Rehabilitation and Recovery
As detoxification preparation, detoxification, aftercare and reintegration are yet done in a non-residential setting, Pre-Treatment and detoxification preparation are of arbitrative significance. It is necessary to go along with the client having the right support system of family members, friends or significant others to hold on and be carried. If needed, a new living environment has to be found together with the client which has a positive effect on the treatment and recovery process, not located in the heart of Mangwe’s (a kind of bar giving out locally brewed palm wine), bars or pubs, in or near a drug den or in a neighbourhood distinct by substance abuse or heavy substance use.
If it is not possible at that very moment to create a support system, special efforts have to be put into strengthening the client. The clients face frequently family rejection due to their substance dependence, also hence the knowledge and the awareness of substance dependence and its components are broadly missing, and most of their "friends" and acquaintances live the same life style. If the client is homeless a respective solution has to be brought about together. If the children of a client are living in the streets, are neglected, engaged in child labor or exposed to other sever dangers and abuse - especially the girls concerning sexual harassment and abuse -, this situation has to be solved together first and protective precautions have to be adopted.
A core principle of amescosa – road to recovery is “give and take”. Our services are not for free. This can though happen symbolically. This is of great importance because otherwise the support will be taken and valued for granted and the necessary efforts and liability will lack. For detoxification we check together with the client on an appropriate caretaker, a familiar and healthy, female person who is close to the client. The caretaker will stay for 24 hours over the 2 weeks of detoxification and the week after with the client at home to give care and support in close team work with amescosa Treatment Centre. If the client cannot get an appropriate caretaker, amescosa has a caretaker on call. The duties of the caretaker are clearly defined, a training takes place and worse case scenarios will be arranged together with the amescosa team. The caretaker is responsible for the care and security of the client, that the client takes the homeopathic remedies as prescribed and the special tailored diet is followed three times a day. The caretaker joins the client to the daily appointments at amescosa Treatment Centre and supports and encourages the client during the detoxification process especially if withdrawal symptoms make the client feel unwell. The caretaker also looks after the household. Through her daily feedback about the personal experience and observations the caretaker contributes vital to guarantee at any time the modulation of the treatment.
Ancillary the Social Worker does two daily home visits in the morning and in the evening and supplies the nutrition in portions for 2 days. During the detoxification and the week after the client's children are not staying at home.
After 1 week, the client’s body is detoxified and she does not experience any or just slight symptoms. From this moment, the therapeutic work begins applying Dance and Movement Therapy, Gestalt Therapy, Therapeutic Improvisation, Drawing Therapy, Breathing Therapy, Imagination, various relaxation methods and Meditation. Simultaneously, also the secured livelihood sustainability will be tackled. Many young women are single mothers and have the sole responsibility for their children. Through a business training the client engages with the business idea, the business strategy, the business plan, the saving plan and the business organization. If everything is developed together with the Social Worker, the clients gets a cash box, is assisted in opening a bank account and receives a single, refundable seed capital in goods. This intervention shall ensure that the client is in a first phase going to be empowered to be able to afford all the basic needs like living, food, water, cloth, children’s care and the necessary additional trifles. If the children are still of school age the saving plan shows a respective item which is first and foremost important if they have no access to education yet. The main idea prospects that the client has to take and fulfil all her responsibilities as a mother and the Social Worker ensures an accurate monitoring and controlling. If the criteria are given a child can be referred to other organizations or services with a scope entirely on education, pressing with secondary school. If possible and wished the client can be supported in assuming the child father in his liability, even though absent. In the course of building a stable financial life foundation we also endeavor to link the client to programs helping her to improve her means gradually.
Apart from the therapeutic single working and various theme groups we offer also couple and family therapy, dependence group and next of kind group.
amescosa-ambassadors build a bridge between amescosa Treatment Centre, potential clients and the community at the gras roots level. amescosa-ambassadors reach out to young women and mothers at public places, Mangwes, pubs, bars or other preferred venues to sensitize on alcohol and poly-drug use disorders, the divers effects and possible support and inform the community at large directly about the services amescosa Treatment Centre provides. Through their movement, the amescosa-ambassadors also discover additional need for interventions and allow our work to be dynamic.
In this effort a cooperative collaboration with the Chiefs of the different Wards in Kilifi, the Village Elders and the Wazee wa nyumba kumi (person responsible for 10 households) is installed.
